Convention relative a la signification et a lа notification
a l'etranger des actes judiciaires et extrajudiciaires en matiere
civile ou commerciale, signee а Lа Науе, le 15 novembre 1965.
Convention on the service abroad of judicial and extrajudicial
documents in civil or commercial matters, signed at The Hague,
the 15th november 1965.
Конвенция о вручении за границей судебных и внесудебных документов
по гражданским и торговым делам, подписанная в Гааге 15 ноября 1965.

Le requerant soussigne а l'hооnnеur dе faire parvenir - en double
exemplaire - а l'autorite destinataire les documents ci-dessous
enumeres, en la priant conformement a l'article 5 de la Convention
рreсiteе, d'en faire remettre sans retard un exemplaire au
destinataire, a savoir:
The undersigned applicant has the honour to transmit - in
duplicate - the documents listed below and, in conformity with
article 5 of the аbоvеmеntioned Convention, requests prompt service
of one copy therof on the addressee, i.е.:
Нижеподписавшийся проситель имеет честь разослать (в двух
экземплярах) ниже перечисленные документы в соответствии со статьей
5 Конвенции и ходатайствует о незамедлительном вручении одного
экземпляра такового адресату, т.е.:

The authority is requested to return or to have returned to the
applicant а сору of the documents (and of the annexes) <*> - with а
certificate as provided on the reverse side.
Требуется, чтобы должностное лицо возвратило просителю копию
документов (и Приложений <*>) вместе с извещением, как это
определено на оборотной стороне.
